KMT2A encodes a transcriptional coactivator that plays an essential role in regulating gene expression during early development and hematopoiesis. Multiple chromosomal translocations involving this gene are the cause of certain acute lymphoid leukemias and acute myeloid leukemias. Patients with KMT2A-rearranged ALL have a poor prognosis. ETV6/RUNX1 is the most common fusion gene in childhood ALL. ETV: This gene encodes an ETS family transcription factor. The product of this gene contains two functional domains: a N-terminal pointed (PNT) domain that is involved in protein-protein interactions with itself and other proteins, and a C-terminal DNA-binding domain.RUNX1: Core binding factor (CBF) is a heterodimeric transcription factor that binds to the core element of many enhancers and promoters. The protein encoded by this gene represents the alpha subunit of CBF and is thought to be involved in the development of normal hematopoiesis. IGH (immunoglobulin heavy chain) is the large polypeptide subunit of an immunoglobulin and located on chromosome 14. FISH can detect the rearrangements of the IGH gene which is associated in many hematologic cancers. ABL1/BCR fusion gene is mainly caused by the t(9; 22)(q34; q11.2) translocation, which results in the Philadelphia chromosome. It is the hallmark in chronic myeloid leukemia (CML), but can also be present in acute lymphoblastic leukemia (ALL) and acute myeloid leukemia (AML), MPD, MDS. It is a useful prognostic factor.
