Hematopathology Peripheral Blood Collection Kit

Available Kit Components:

  • 2 Slide holders
  • 4 beveled-edged slides.
  • 1 Biohazard bag size (8” x 10”)
  • 1 Ice gel pack
  • 2 Green top tubes (Sodium Heparin 12mg)
  • 2 Purple top tubes (K2 EDTA 7.2mg)

  • Hematopathology Requisition Form
  • 1 FedEx Clinical Pak with FedEx label

Instructions

Peripheral Blood Submission: Lavender Top (Flow, Molecular) or Green Top (Cytogenetics, FISH)

Air dried smears are highly desirable but not mandatory.

Place specimens in foam insert, cover with absorent materials & seal absorbent bag.

Please include completed requisition, insurance information, current CBC (within 30 days), and all clinical notes.

Scan the siPtrack barcode on the box for pickup or call Client Services 888.599.5227

Please place the collection kit in the FedEx Clinical Pak.


Important Notes:

Tests preferentially performed on Green Top tubes: Cytogenetics (chromosomal karyotype and FISH).

Tests preferentially performed on Lavender Top tubes: Flow Cytometry and Molecular Tests for Hematologic Cancers.

Please note: Peripheral blood "Liquid Biopsy" molecular tests for solid tumors must be submitted separately using a siParadigm Liquid Biopsy Collection Kit.
