Precision Oncology Fresh Tissue Collection Kit
Available Kit Components:
- 1 Slide holder
- 1 AffirmSeal jar
- 5 Slides with beveled edge
- 1 Biohazard bag size (8” x 10”)
- 1 Ice gel pack
- 1 Purple top tube (K2 EDTA 7.2mg)
- 1 Polystyrene conical tube 15 mL
- Solid Tumor Requisition Form
- 1 FedEx clinical pak with label.

Instructions

Obtain three needle passes for:
- Slides (≥2 air-dried and ≥2 immediate alcohol-fixed). Place in respective slide holders,
- RPMI tube for flow and/or molecular studies (keep refrigerated), and
- Thin-prep tube (Cytolyte).

Peel off label from the requisition and place on RPMI tube.

Place specimens in foam insert, cover with absorbent material & seal Ziploc bag.

Place a cold pack outside sealed Ziploc bag if >80°F.

Include requisition, billing info, and EMR notes outside Ziploc bag.

Scan the sipTrack barcode on the box with tablet scanner for pick-up confirmation. Otherwise, call 888-599-5227,ext.#2 for pick-up.

For FedEx service: Place box in FedEx clinical pak.
